What's the difference between the two plans?

Peak Health is designed for proactive optimization and longevity, as well as single-focus health concerns at $3,500 per year. The Transformation plan is for complex chronic conditions at $7,000 per year with direct care from Dr. Mote. Both utilize advanced diagnostics, the Longevity Assessment, Functional Medicine, an interdisciplinary care team and personalized protocols.

Can I switch between plans?

Yes. If your health needs change, you can transition between plans. We recommend discussing this at your initial visit so we can adjust your care accordingly.

What insurance payors do you accept?

Cornerstone is enrolled in most insurance plans and accepts Medicare. Some plans require that your PCP be part of a hospital-owned group (we’re a private practice), so call your insurance company’s membership services line to verify that we are in-network.

Are the membership fees covered by insurance?

No. Membership fees are for non-billable, non-insurance eligible services. They can be HSA/FSA eligible. Please speak with your plan administrator for specific details.
