Cornerstone is in-network with most major insurance plans. To verify that Cornerstone Health Community is an in-network PCP, please call the member services line listed on the back of the insurance card and present the representative with our Group NPI number: 1003273293.
Many members use their FSA or HSA funds to pay for their membership and related costs like specialty testing, and supplements. We always recommend checking with your FSA or HSA provider first to confirm what's eligible and learn how to submit documentation.

4. Complete your health history
Complete your health history and office forms digitally through our secure platforms. We also guide you through the process of transferring any medical records to our office.
5. Schedule your first appointment!
At your first appointment, one of our board-certified providers will listen to your health story and help formulate a comprehensive plan for your goals. This will be a fasting appointment, as we will run a blood test with 75+ biomarkers.
