Here is what your membership experience looks like.

Step 1

Record your comprehensive health story

Prior to your first visit, you will complete a digital intake questionnaire covering your family history, symptom history, medical background, lifestyle and health goals through a functional medicine, longevity science and trauma informed approach. You also submit any prior labs and tests to your patient portal. Your board-certified clinician reviews it all before your initial visit so that your initial face-to-face visit starts with a robust dialogue.

Plan to spend about 30 minutes completing the health story intake. Many members say this is the first time they feel that their health journey and life story is being contextualized and centered within a healthcare setting.  

Step 2

Meet your board-certified clinician for a personal initial visit & establish your baseline

Your first visit with your clinician is equal parts listening and thoughtful questions. We review your intake in advance and prepare for theappointment with great care. Together, we go deep into your history, your labs, your habits, your concerns. Your health goals are our shared passion. We share root cause thinking.

Plan to be fasting. Our skilled phlebotomist will draw your blood in-house. Far beyond a standard annual lab panel. We assess comprehensive metabolic function, hormonal status, thyroid health, inflammatory burden, cardiovascular risk markers, nutritional status, immune function, and organ system health — building a complete biochemical picture of your biology.

The first visit will be in person and about 60 minutes in office.

Step 3

Measure your healthspan markers

You will be guided by our highly trained technicians through a three-pronged, in-office, longevity science assessment. It is a state-of-the-art, multi-dimensional evaluation of your current biological status and future health trajectory. Each component is selected because it provides actionable, evidence-based insight into a specific dimension of how you are aging — and how we can optimize it. Our longevity assessment is designed for early detection and empowerment.

Step 4

Review your plan

The assessment is only the beginning. Every finding — every biomarker, every test result, every imaging study — informs a personalized, integrated Care Plan built around your biology, your goals, and your life. As we craft your personalized plan, we draw from the full depth of our clinical capabilities and team insights. During this hour-long appointment with your provider, the plan takes root and we go in even deeper to find the why behind the symptoms.

Step 5

Meet your nutrition coach, excercise physiologist, and more

You will complete five days of lifestyle and nutrient data. We look for patterns and sensitivities. We review your body composition data. You share your movement history, cardiovascular activity and strength building goals. We discuss circadian rhythms and name the daily changes that support your care plan. We have a trauma-informed approach and care for your body, mind and spirit knowing that these topics can be embedded in story and past pain.

Step 6

Ongoing partnership and care

Your health is not a destination — it is a practice. Each visit is structured as a continuous, evolving partnership between you and your care team. Your clinician will look at your wearable data, add specialty testing, adjust your protocol, and refine treatments as you make progress against your baseline.
